We do not accept New or Transfer High School students past the 10th grade.
You must contact the Homeschool Coordinator after reading through this packet overview to receive the link to the registration forms and the password to access them.
Here is an explanation of the registration forms. We do not look favorably on tardy paperwork so please pay close attention to the dates on which these forms are due.
This check list is not for you to print and fill out but for the coordinator. You do not need to print it off. It is included so that you can see all the things you will need to bring with you to the final meeting. One thing you will notice on the check list is that a copy of the teaching parent's high school diploma must be turned in at the final meeting. This is a State requirement.
This form must be printed off and completed for the final meeting. Please be sure to fill in every field on this form. The bottom of the form does not always print off completely but there is space at the bottom where you need to sign and date the document. On this form is a space for your email address. The area provided has caused problems in the past because most email addresses are very long and have to be compressed to fit. However all communication from this Umbrella will be through email and so a correct email address is pertinent. Please send the Home-school coordinator an email the day of your meeting stating the email address that you would like to be entered in and used to communicate with you so that we can be sure it is entered in correctly. We have had email addresses entered in incorrectly in the past and students have missed important information that was sent through mass emails.
Statement of Faith:
This form must be printed off and completed for the final meeting. Please be sure at least one parent signs and dates at the bottom. If two signatures are possible that is better.
Certificate of Immunization:
This is an official form and must be filled out by a certified Health-care provider. You can print out this form or use the form that they have. Printed proof of immunization must be turned in at the final meeting. If you do no vaccinate for religious reasons print this form off and please fill in the information on the top left of the form. Then check the religious exemption box on the top right of the form. Please sign and date the form at the bottom right to verify that you are choosing not to vaccinate.
This form will be filled in at the beginning of each semester and turned in to the Homeschool Coordinator on the last day of each semester. For second semester it must be turned in by June 30th. This form will show the classes taken and the grades received in those classes which will be transferred to the transcript for your student. The dates that the curriculum worksheet is due are shown on the News & Events Calendar. A $20 late fee will be charged for each week that you are late turning in this form. After four weeks late your student will not receive credit for the classes taken during that semester and will be required to repeat them.
Financial Enrollment Contract:
This form must be printed and completed for the final meeting. Full payment for the year will be required at the final meeting. Notice that graduation for 12th grade is optional and is additional $250. Please notify the Homeschool Coordinator at the time of registration if your senior will be participating in graduation. The Co-curricular and Fine Arts fees are also optional. These fees are further explained in the Home-school overview and on the services page. Please pay by check if possible. Credit or Debit will only be an option if the school business office is open at the time of your meeting.
Please use the 2018-2019 Fee Schedule to determine if you are eligible for a Registration discount.

Pastoral Reference Letter:
This form must be printed and completed for the final meeting. This is a special form required of all new King's Academy High School students both in the day school and the Homeschool Umbrella. Please note this form is to be filled out by a Pastor only.
This form is to filled out and turned in to the Homeschool Coordinator on the last day of each quarter. For 4th Quarter it must be turned in by June 30th. This will help keep your student on track to receive the correct hours and credits for each year. The dates that the attendance report is due are shown on the News & Events Calendar. A $20 late fee will be charged for each week that you are late turning in this form.
This form is to be filled out throughout the school year to keep track of the 180 days of annual attendance required by the State. This form must be turned in to the Homeschool Coordinator by June 30th or as soon as 180 days have been reached. This date is shown on the News & Events Calendar. A $20 late fee will be charged for each week that you are late turning in this form.
Teacher In-Service Form:
This form is to be filled out during the school year. This is to show different ways that the teaching parent has improved their knowledge through the year to better teach their student. It may be an article, convention, website, program, etc that helped to better equip you as a teacher. This form will be turned in to the Homeschool Coordinator by June 30th to complete your file for the year. A $20 late fee will be charged for each week that you are late turning in this form.
Community Service Form:
This form is to be used to keep track of all community service hours logged during High School. These hours can be useful when applying to a University. This form can be turned in to the Homeschool Coordinator to be kept in the student's file, or can be kept by the parent according to your preference. This form is to be filled out by the team, supervisor or organization where the service hours were performed.
Time Log Form:
This form is be used to keep track of hours fulfilled towards a P.E, Fine Arts, or Elective class credit. This form must be turned in at the end of the semester that the class was taken in to show how many credits have been earned.
