New or Transfer High School students past the 10th grade will only be accepted on a case-by-case basis after careful review of the official transcript.
You must contact the Homeschool Coordinator after reading through this packet overview to receive the link to the registration forms and the password to access them.
Here is an explanation of the registration forms. A late fee is charged on tardy paperwork so please pay close attention to the dates on which these forms are due.
Check List:
You do not need to print off this check list - it is for the coordinator to use. It is included so that you can see all the things you will need to bring with you to your registration meeting. You will notice on the check list that a copy of the teaching parent's high school (or college) diploma must be turned in and kept in your student's file. This is a State requirement.
Application Form:
This form must be printed off and completed for the registration meeting. Please be sure to fill in every field. The bottom of the form does not always print off completely but there is space where you need to sign and date the document. On this form is a space for your email address. The area provided has caused problems in the past because many email addresses are very long and have to be compressed to fit. However, all communication from this Umbrella will be through email and so a correct email address is pertinent. Please send the Homeschool coordinator an email the day of your meeting using the email address that you would like to have on record so that we can be sure it is entered in correctly. We have had email addresses entered incorrectly in the past and parents have missed important information that was sent through mass emails.
Statement of Faith:
This form must be printed off and completed for the registration meeting. Please be sure at least one parent signs and dates at the bottom.
Certificate of Immunization:
This is an official form and must be filled out by a certified health care provider. You can print out this form or use the form that they have. Printed proof of immunization must be turned in at the registration meeting. If you do no vaccinate for religious reasons please print this form off and fill in the information on the top left of the form, then check the religious exemption box on the top right. Please sign and date the form at the bottom right to verify that you are choosing not to vaccinate.
Curriculum Worksheet:
This form must be printed off and completed for the registration meeting to indicate the courses and curriculum you have chosen. You will also complete this form at the beginning of each semester and turn it in to the Homeschool Coordinator on the last day of the fall semester, and no later than June 30th for the spring semester. This form will show the courses taken and the grades received, which will be transferred to the official transcript for your student. The specific due date for the Curriculum Worksheet for fall semester is shown on the News & Events Calendar. A $20 late fee will be charged for each week that you are late turning in this form. After four weeks late your student will not receive credit for the classes taken during that semester.
Financial Enrollment Contract:
This form must be printed and completed for the registration meeting. Notice that graduation for 12th grade is optional and has an additional $250 fee. Please notify the Homeschool Coordinator at the time of registration if your senior will be participating in the TKA graduation events. The Fine Arts fee is also optional and is further explained in the Homeschool Overview and on the Services page. If you are enrolling more than one child, please use the 2020-2021 Fee Schedule to determine if you are eligible for a registration discount. Full payment for the year will be required at the final meeting. Please pay by check if possible. Credit or Debit will only be an option if the school business office is open at the time of your meeting.
Student Questionnaire:
This form must be printed and completed for the registration meeting. It is required of all new King's Academy High School students, both in the day school and the Homeschool Umbrella.
Pastoral Reference Letter:
This form must be printed and completed for the registration meeting. It is required of all new King's Academy High School students, both in the day school and the Homeschool Umbrella. Please note this form is to be filled out by a Pastor only.
Quarterly Attendance Report:
This form is to be filled out and turned in to the Homeschool Coordinator on the last day of each quarter. For 4th Quarter it must be turned in by June 30th. This will help keep you and your student on track to complete the correct hours and credits for each year. The dates that the attendance report is due are shown on the News & Events Calendar. A $20 late fee will be charged for each week that you are late turning in this form.
Attendance Summary:
This form is to be filled out throughout the school year to keep track of the 180 days of annual attendance required by the State of Tennessee. This form must be turned in to the Homeschool Coordinator by June 30th or as soon as 180 days have been reached and you have finished the school year. A $20 late fee will be charged for each week that you are late turning in this form.
Parent/Teacher In-Service Form:
This form is for parents to use during the year to show different ways that they have improved their knowledge through the year to better teach their student(s). It may be an article, convention, website, book, Podcast, etc that helped to better equip you as a teacher. This form will be due on June 30th, along with your Attendance/Grades form to complete your file for the year.
Community Service Form:
This form is to be used to keep track of all community service hours logged during High School, and should be filled out by the team, supervisor or organization where the service hours were performed. These hours can be helpful to include on college and scholarship applications. These forms can be turned in to the Homeschool Coordinator to be kept in the student's file or kept by the parent, according to your preference.
Time Log Form:
This form is be used to keep track of hours fulfilled towards P.E, Fine Arts, or Elective class credits. Time logs must be turned in at the end of the semester that the course was completed to show how many credits have been earned.
65 hours are required to earn .5 credit, and 130 hours for a full credit.
